Autism and ADHD sensory differences, explained

Sensory difficulty is one of the most talked-about experiences in both autism and ADHD — and one of the most commonly confused. A child who covers their ears at a shopping centre, or an adult who can't concentrate in an open-plan office, could fit either pattern. The difference isn't whether the senses are involved; it's the mechanism underneath. Getting this distinction right is genuinely useful, because it points in different directions for what actually helps day to day.

Intensity-driven (autism-pattern)

In an autism-trait pattern, sensory difficulty is about intensity. Specific stimuli — a particular pitch of sound, fluorescent flicker, the texture of a clothing tag — register more strongly than they do for most people, to the point of becoming genuinely overwhelming rather than merely annoying.

Two tells point to this mechanism: distress scales with the intensity of the input, and unpredictability makes it worse. Intensity someone chose and controls is far more bearable than intensity that arrives by surprise. After a sensory-heavy day, the recovery need is usually quiet, dim, and alone.

Attention-driven (ADHD-pattern)

In an ADHD-trait pattern, the same busy room causes a different problem. The input doesn't overwhelm so much as hijack: every new sound or movement pulls attention away, and the thread of what you were doing is lost. The volume isn't the wound — the grip on your attention is.

The other tell is the opposite of the autism pattern: the aversive state is under-stimulation. Quiet, slow, monotonous situations feel almost unbearable, which is why someone with this pattern goes looking for stimulation — noise, movement, novelty — rather than away from it.

Seeking behaviour cuts both ways

Both patterns can involve actively seeking sensory input, which adds to the confusion — but the reason differs. An autism-trait pattern seeks input to regulate: deep pressure, repetition, the same song on a loop, because sameness settles the system. An ADHD-trait pattern seeks input to stimulate: new, different, more, because a quiet system feels like static.

Not noticing input at all

Both patterns can also involve under-registering sensations, but again for different reasons. An autism-trait pattern may genuinely not notice hunger, thirst, or temperature until it's extreme. An ADHD-trait pattern more often misses input during hyperfocus — when someone is deeply absorbed, the doorbell, their name, and the burning toast simply don't land.

Why the distinction matters

The mechanism points to different day-to-day approaches. Where the issue is intensity, reducing and making input predictable tends to matter most. Where it's attention, managing distraction and building in stimulation tends to matter more. And many people carry both mechanisms at once — which is why a good screening conversation measures each independently rather than assuming one story.

Can sensory issues exist without autism or ADHD?

Yes. Sensory sensitivity and sensory-seeking occur across the whole population and are part of ordinary variation. They are only one piece of a much broader trait picture, which is why no single sensory experience points to a particular neurotype on its own.

My child seeks and avoids sensory input — is that contradictory?

Not at all — it's common. Someone can be overwhelmed by one kind of input (say, unexpected loud noise) while actively seeking another (deep pressure, movement). Mapping which inputs do which is more useful than a single label.
